2. HOW TO PRINT OUR ORDER FORM


When the TEXT FORMAT pops up on your screen you can print it right away.
Just click the word "FILE" in the upper left corner
Then click "PRINT"


You can, of course, save the Order Form on your hard drive
and print it at a later time.

3. FILL OUT & MAIL THE THE FORM


After printing the Order Form on your printer:

You can type in your order or fill it out by hand

Please use a PEN rather than a pencil

Fill out an Order Form for EACH candle ordered

Add Shipping Costs (see below) and applicable Sales Tax

Mail the Order Form to us along with your payment

Please allow 4--6 weeks for delivery

SHIPPING & HANDLING RATES & POLICIES:

We ship Priority Mail and UPS all over the United States and in many different shipping zones. We've found it is much easier to "guesstimate" shipping costs to initiate your order. You may receive a refund in the mail for any overpayment. Please use this simple chart below to guesstimate your Shipping and Handling costs:
